Stemming from newly enforced Federal tax laws, we are required by law to maintain a complete,
up-to-date file of exemption certificates for our customers to substantiate the exempt sales we report to all states.

As of April 1st, sales tax will be charged on all future orders unless a Tax Exemption Certificate is filed for your company. Please complete and sign the attached exemption, listing your registration certificate(s) or permit number(s) for all states in which you claim exemption. You can also provide your own completed exemption certificate. A picture of your tax permit is not considered a legitimate exemption certificate by states. It is required to have your number and be dated and signed by you.

SPM is required to charge sales tax on all orders for customers for which we do not have a valid resale
exemption certificate on file.  If we do not have a valid form on file for your company, it is urgent that you to provide completed exemption forms prior to your next order.

If you have already submitted this form to us prior to receipt of this documentation, please disregard this notice. If you are not sure, it is better to have sent us two than none.
